Consumer Directed Services

This program is intended to help prevent the need for nursing home placement by providing at-home assistance for those who are physically disabled.

Finding the right caregiver to support you with your daily tasks is important. You can feel comfortable and safe when you trust the person who will provide you with assistance. That is why we offer the CDS service to our clients. This is a Medicaid program wherein you can hire a friend or family member to care for you; however, you cannot enlist the assistance of your spouse, legal guardian(s) or any individual below 18 years of age.

CDS also referred to as self-directed care, eligible applicants can hire, train, and supervise, the individual(s) they choose to provide their personal care.

To be eligible for this service, you must be:

• Medicaid eligible - enrolled in MO HealthNet
• Mo resident 18 years of age or older
• Able and willing to direct their own care
• Physically disabled
• Able to live independently
• Unable to perform daily routing activities
• Must require level of care that is provided in nursing home
